A little preparation helps the appointment go smoothly.
- Please bring a photo ID and your insurance card if you have one.
- Many visits take about 45–90 minutes. Your appointment instructions will confirm timing.
- If we sent forms ahead of time, complete them if you can. If not, we’ll help in the office.
We do not collect records, medications, or insurance numbers in this app.
